He is survived by: his father Thomas David Church Sr.; his wife Dawn Church; his children, Abby Church and Miles Church (Starr); his pet Huxley; his siblings, Thomas David Church Jr. (Jennifer) and Catherine Wilson (Jason); and his parents-in-law, Larry and Connie Parrish. He is also survived by other family members and friends.
