He was predeceased by: his parents, Henderson Forrest and Dale Forrest; his sister Myrna Gail; his aunts and uncles, Peppi, Ribbi, Brother Joe, Brother Nelson, Bubba Bill, Duke and Carlton; his niece Terri Lynn; his grandparents, Birdie, John, Andrew and Nannie; his brother-in-law Harold "Swabby" Rathbone; and his close friend Robert "Cut" Firth.
He is survived by: his wife Theresa; his children, Amber, Charlie, Thomas and Keith; his pet Bentley; his aunt Harriet Lane; his siblings, Anne Nelson and Henderson Sr.; his siblings-in-law, Frank and Robin; his nieces and nephews, Beth, Dave, Jason, Lisa, Joanne, Henderson Jr., Robyn, Andy, Billy, Kelli, Grayson and Tricia; and his friends, Fimiah Lou Pauls and Ann Leone Forrest.
