She was predeceased by: her father Robert Clark Forman; and her sister Jacqueline Hart. She is survived by: her mother Susie Hart Forman; her children, Ashley Slate McGuire (Devin), Chelsea Slate Weiskopf (Trey), Monica Dawn Slate and Marcus Daniel Slate; her grandchildren, Whitney, Landon McGuire, Windsor, Leighton Weiskopf, Nicholas Slate, Peyton and Reagan Slate; her husband Gil; and her brothers, Robert Clark "Tede" Forman and Charles Edward "Chas" Forman. She is also survived by her daughters and sons-in-law; her four grandchildren.
A memorial gathering was held on Thursday, August 27th 2026 from 4:00 PM to 6:00 PM at the Woody Funeral Home-Parham (1771 N Parham Rd, Richmond, VA 23229). A celebration of life was held on Friday, August 28th 2026 at 11:00 AM at the River Road Baptist Church (8000 River Rd, Richmond, VA 23229).
