He was predeceased by: his wife Sadako Leftwich. He is survived by: his sister Debra Horn; his sons, Frank W. Leftwich (Pam Leftwich) and John M. Leftwich (Sara); his grandchildren, Brian (Rachel), Brooke (Chris), Brittany (Waylon) and Jeremy Leftwich; and his great grandchildren, Owen and Gabriel.
