He was predeceased by: his parents, Walter Cook, Jr. and Mayfield New Cook; and his siblings, Walter Cook, III, Willie Cook, Mike Shelton, Randy Shelton, Gloria Jean Jones and Brenda Throckmorton.
He is survived by: his children, Jonathan Cook and Mikaela C. Metz (Stephen); his siblings, Joyce Thomas, Tim Shelton (Marilyn), Les Cook (Beverly) and Dana Cook (Debbie); and his close friend Cricket Miles. He is also survived by numerous nieces & nephews and the mother of his children.
