He was predeceased by : his parents, Jesse Cook and Elaine Cook; his siblings, Herbert Cook, Joe Cook, Verna Mae Reid and Edith Payne; and his nephew Justin Cook.

He is survived by : his wife Brenda Watts Cook; his children, Chris Cook (Danielle), Jason Cook (Kim) and Teresa Cook; his grandchildren, Christopher, Devin, Abigail, Bodie, Kendra and Hope; and his siblings, Howard Cook, Virginia Myers (Tommy), Patricia Monteith, Brenda Cook and Patricia Crose. He is also survived by many nieces and nephews.
