He was predeceased by: his father John Kulakowski. He is survived by: his wife Joyce Kulakowski; his daughter Lori; his son-in-law Ian McDonald; his daughter Lisa Kulakowski; his son-in-law Jon Bivens; his grandchildren, Alex (Sarah), Eric, Lindsay, Henry, Keaton and Libby; and his sisters, Janette Jordan (Ray), Christina Costa, Caroline Oder (John) and Barbara LaFreniere. He is also survived by several nieces, nephews, cousins, and many dear friends.
In lieu of flowers, memorial donations made be made to a Parkinson's/Dementia Foundation of choice.
