He was predeceased by: his parents, Rex G. Evelsizer and Pearl Lucille Evelsizer; his sister Dona Jean Evelsizer Hissom; his brother-in-law Robert Hissom; his brothers, Rex G. Evelsizer, Jr. and Robert Edward Evelsizer; and his sister-in-law Geraldine S. Evelsizer.
He is survived by: his wife Donna Lea Weaver Evelsizer; his children, Michelle Evelsizer Peck (Jeffery) and Michael Lyle Evelsizer (Misty Kyle); his grandchildren, Alexander Michael Peck, Caleb Nelson Brink Peck, Taylor Faith Evelsizer Swisher (Josh Swisher) and Jayden Diane Evelsizer; his great grandchild Maci Dawn Swisher; and his sister-in-law Donna Jean Evelsizer of Ohio. He is also survived by several nieces, nephews and his friends.
