He was predeceased by: his parents, Frank O'Roark and Marjorie O'Roark; his parents-in-law, John and Evelyn Foster; and his son Howard Talley Cook, Jr. (Rick).
He is survived by: his wife Earleen; his children, Amy Driscoll (Kenny) and James "Jay"; his grandchildren, Courtney Thompson (Mike), Tiffany King, Richard "Bodie" Driscoll, Jadyn Cook and Kalia Cook; his great grandchild Wyatt Thompson; and his sister Marsha Watson. He is also survived by many nieces and nephews; and lifelong friends.
