He was predeceased by : his mother Pauline Stone. He is survived by : his father Robert Polhemus; his wife Cindy; his children, Mallory, Michael and Candace; his siblings, Stormie Polhemus and Chuck Polhemus; his grandchildren, Morgan, Madison, Kinley, Kai, Emma and Eli; and his great grandchildren, Brantley, Kevin, Kayden and Bailey. He is also survived by numerous nieces, nephews and cousins.

In lieu of flowers, Chip expressed a desire that donations be made to the Franklin County Long Beards Chapter of the National Wild Turkey Federation-NWTF.
