He was predeceased by : his parents, John E. Provasnik and Irene Hagan. He is survived by : his wife Sharon McNamee Rockwell; his sons, Kevin Provasnik, Gregory Provasnik, Daniel Provasnik and David Provasnik; his stepdaughter Kathleen Rockwell (Katie); and his caregiver Irv.
In lieu of flowers, the family requests memorial gifts be sent in Irv's honor to Fred Hutchinson Cancer Center Patient Care Fund.
