He was predeceased by: his parents, Elaine Cornwall (Prince) and Jean Cornwall; and his brothers, Mark and Steven Cornwall. He is survived by: his wife Linda Kotsch; his daughters, Meghan French (Justin), Becky Cornwall and Caitlin Donigian (Alex); his stepson Gabe Kotsch; his grandchildren, Ashley, Riley, Zoey and Addie; and his sister Robin Richardson. He is also survived by many nieces and nephews.

Memorial donations in honor of David may be made to Seattle Children's Hospital or St. Jude Children's Hospital.
