He was predeceased by: his parents, Raymond Cook and Lila Cook; his sister Linda Rodgers; his niece Katie Rodgers; and his sister-in-law Kristi Cook.

He is survived by: his daughter Cori May Cook; and his siblings, John Cook, Barbara Sperry (Dennis), Marty Cook and Judy Murray.
