He was predeceased by : his parents, John Kotlan and Eunice Kotlan; and his sisters, Jean, Viola and Joan.
He is survived by : his wife Laura Kotlan; his children, Allen Kotlan (Susan) and Lorie Ann Mack (Brien); his grandchildren, Sarina, Desirae and Tiffany; and his brother Marvin Kotlan (Hazel).
In lieu of flowers memorial contributions may be made to St. Jude Children's Research Hospital in care of Keith & Keith Funeral Home.
