He was predeceased by: his parents, William Dvorak and Clara Dvorak; his former wife Jean Fallon; his wife Sharon Dvorak; and his siblings, Dennis Dvorak, David Dvorak and Doreen Rockett.
He is survived by: his children, Donna Halleran (Scott), Ken Dvorak (Janelle), Bob Dvorak (Cary), Sharon Jameson (Carl), Ron Dvorak and Richard Dvorak (Olivia); his grandchildren, Will, Nina, Conor, Caitlin, Trevor, Ryan, Meghan, Benjamin and Diego; his great grandchildren, Nila and Elena; and his brother Daryl Dvorak (Beverly).
Visitation will be held on Wednesday, July 2nd 2025 from 9:30 AM to 11:30 AM at the St. Pius X Catholic Church (500 W Marquette St, Appleton, WI 54911). A Mass of Christian burial will be held on Wednesday, July 2nd 2025 at 11:30 AM at the same location.
