He was predeceased by: his parents, Edward Slavik and Loretta Slavik (Dimmer); and his brother Lawrence (Elaine). He is survived by: his children, Donald (Cindy Barfknecht), Jonathan "Jes" (Eileen Mullinaux), Laure Alwin (Todd), Daniel (Laura Van Cuyk), Ted (Vicki Papez) and Lisa Lambie (Kevin); his grandchildren, Joseph Lambie (Tammi), Samantha Jones-Dewall (Jake), Jonathan Slavik (Kate), Jennifer Spagnolia, Amy Reilly (Brandon), Katrina Slavik (Gary Shlahtichman) and Kailey Slavik; and his great grandchildren, Kinsley, Grant, Erin, Michael and Aiden.
In lieu of flowers, a memorial fund has been established.
