He was predeceased by: his parents, David L. Smith and Alyce L. Smith (Oosterhouse). He is survived by: his wife Ann Elizabeth Ottersen; his children, Kristen Souza (Nick), Amy Barrett (Zach) and David Smith (MJ); and his siblings, Matt (Jan), Elizabeth (Randy), Leigh (Beth) and Stuart (Tammy). He is also survived by many nieces and nephews.
