He was predeceased by: his parents, Daniel Lizak and Margaret Lizak (Berhow); and his siblings, Bill, Wally Lizak, June Moore and JoAnne Lizak.
He is survived by: his wife Lori; his children, Kenneth Lizak (Cindi), Kristofer Lizak (Wendy) and Victoria Lizak-Lund (Carter); his grandchildren, Colton Lizak (Winnie Pye), Calysta Lizak, Stephanie Lizak (Brandon Moore), Alexander Lizak (Carissa Levinski), Rylan Lizak, Dylan Lund and Lucas Lund; his great grandchild Mazikeen Moore; and his siblings, Edward Lizak (Julie), Doug Lizak (Diane), Helen Franek and Millie Godfrey (Bruce). He is also survived by numerous nieces and nephews.
In lieu of flowers, memorials may be sent to the family to help with expenses.
