He was predeceased by: his parents, Theodore Jack and Caroline Jack (Pope); his son Nathan; and his brother Thomas Jack. He is survived by: his wife Debra Johnson; his children, Brendon Jack (Jenna) and Neil Jack (Jean); his grandchildren, Trenton Machiela, Jayden Jack, Jesse Machiela, Ian Jack, Nakota Jack and Kierra Jack; and his sisters, Janet Yuhas (Gene) and Juliet Jack. He is also survived by nieces and nephews.
