He is survived by: his children, Charles Dvorak (Jessamyn) and Pamela Freeze (Scott); his siblings, Cheryl McIntire (Greg) and Carl Dvorak (Terry); his stepmother Donna Dvorak; his stepsiblings, Debbie Sale and John Kowalke; his grandchildren, Alexis, Hannah, Jemini, Lana and Clayton; and his great grandchild Ellianna. He is also survived by nieces, nephews, and cousins.
A funeral service was held on Monday, October 19th 2026 at 11:00 AM at the Spring Brook (242 Lake Shore Dr, Wisconsin Dells, WI 53965).
