He was predeceased by: his parents, Orville Cook and Ethel Cook (Hayes); his son Tommy; and his siblings, Alvin, Roger and Sharon.
He is survived by: his wife Peggy Ann; his daughter Peggy Sue; his niece Cindy Sedbrook; his siblings, Orvie Cook (Nancy) of Fox Lake, Orville Cook (Ann) of Lone Rock, WI and Linda Cook of Dubuque; and his close friends, Mike and Max of Beaver Dam. He is also survived by nieces, nephews, other relatives, and friends.
