She was predeceased by: her father Joseph Michael; and her nephew Nicholas Geier. She is survived by: her mother Irene Daniels (Rundle) of Madison; her children, Thomas Underkofler (Tricia) of Madison, Michelle Schmitz (Jeff) of Appleton, Sara Williams (Sheldon) of Johnson Creek and Jason Underkofler (Tara) of St. Charles, IL.; and her grandchildren, Evelyn, James Underkofler, Caitlyn, Drew, Owen Schmitz, Tia (Jonah), Maya, Qadry Williams, Haley, Aubrey and Lincoln Underkofler.

In lieu of flowers, memorials may be directed to the Make-A-Wish Foundation, in Jackie's name.
