He was predeceased by: his parents, Francis Cook and Margaret Cook (McKinney); his sister Patricia Engendorf; his sister-in-law Karen Cook; and his grandson Joshua Cook.
He is survived by: his grandson Storm "Stormcat"; his children, Kevin Cook of Janesville and Dawn McKinney of Washington; his grandchild Harley Cottrille; and his brothers, James Cook (Marilyn) of Evansville and Steve Cook of Strandburg. He is also survived by five great grandchildren.
