He is survived by: his mother Polly Ann Cholka Sr. (O'Brien); his children, Joey Cholka and Amber Cholka; and his siblings, Rosemarie Adams, Casie LaMorie, Toni Kachnowski, Donald Cholka Jr., Denise, Steven, Phillip and Harry John Cholka. He is also survived by many nieces, nephews and cousins, aunts and uncles.
