She is survived by: her sons, Clark and Troy; her husband Jim Slade; her son Rick; her siblings, Lorrie, Jack and Jill; her grandchildren, Ashly, Montanna and Anson; and her great grandchild Patience. She is also survived by grandchildren, great grandchildren, cousins, nieces and nephews.
