She was predeceased by: her parents, Forrest Ellsworth and LaRheta Ellsworth (Austin); her parents-in-law, Bill and Blondie Shultis; her sister-in-law Elizabeth Shultis; and her nephew Keith Shultis.
She is survived by: her husband Gary W. Shultis; her children, Tina and James; her siblings, Susan Smiley (Mark), Lisa Ellsworth (Tim Mahnke) and Forrest Ellsworth; and her uncle Kenneth. She is also survived by many nieces and nephews.
