He is survived by: his son Matthew Mayotte (Kelly) of Winneconne, Wisconsin; his grandsons, Owen, Rucker and Bodie; his siblings, Craig Mayotte (Linda), Linda Skarda, Rene Walker and Laureen Hayden (Jim); his nieces and nephews, Megan, Aimee, Ashley, Amanda, Kim, Alex, Andrea, Alexa and Arika; and his close friends, Pat, Lori Johnson and Peggy Stofferan.
The family respectfully asks for flowers not to be sent, but rather memorial donations be made to be used for the benefit of his grandchildren.
