He was predeceased by: his parents, Carl Kolarik and Mary Kolarik (Cherney); his wife Laureen (Kafka); his sister Janice Kuehl; his siblings-in-law, Norbert Shefchik, Mary Ann Mincheski, Bonnie Gauthier and Denis Kafka; and his nephew Paul Shefchik.
He is survived by: his children, Jayme of Woodbury, MN and Joni of Denmark; his siblings, Mary Jo Shefchik, Robert Kolarik (Terese) and Bill Kolarik (Kandi); and his siblings-in-law, Maynard Kuehl (Ethel), Jean Minten and Larry Kafka (Donna). He is also survived by Many nieces, nephews, family and friends.
