He is survived by: his wife Linda Phetteplace; his children, Scott Phetteplace (Tina), Lisa McAdams (Tina), James Phetteplace, John Polkinghorn (Susan), Shawna Fromfeld (Andy), Erin Smith (Tony) and Suzie Jacobs (Keith); and his siblings, Robert, Daniel and Linda.
