He was predeceased by: his parents, Francis O'Toole and Sadie O'Toole; his former wife Kathy O'Toole; and his brother John O'Toole (Jack). He is survived by: his wife Kathryn Laura Seratt of Chicago; his children, James J. O'Toole of Minneapolis, MN and Laura Cichocki (Jeff); his grandson Eric Cichocki of Burlington, WI; and his brothers, Robert O'Toole (Bebe) of Bonita Springs, FL, Thomas O'Toole (Elaine) of Lombard, IL and Joseph O'Toole (Bernice) of Naperville, IL. He is also survived by nieces and nephews.
