He is survived by: his wife Kristine "Kris" Polenz; his children, Kyle Polenz (Stephanie) and Amy Johnson (Mark); his brother Orin "Mouse" Polenz; and his grandchildren, Serena Short, Jacob Russel, Mykal Polenz and Bralynn Andersen. He is also survived by many nieces, nephews, extended family members and dear friends.
