She was predeceased by: her parents, George McGinty and Lucille McGinty; and her sister Carol McGinty (Jerry). She is survived by: her husband Jerome "Jerry" Jacques; her daughters, Cecilia Galateo (Thomas) of Burr Ridge, IL, Leslie Bates (Jeffrey) of Eau Claire, WI and Lynn Prueher (David) of Appleton, WI; and her grandchildren, Larry Galateo (Wendy) of Willow Springs, IL, Amy Caronchi (Anthony) of Willow Springs, IL, Mia Channell (Dave) of Willow Springs, IL, Tom Galateo of Lemont, IL, Alex Bates of St. Paul, MN and Sydney Bates of Eau Claire, WI. She is also survived by her nine great-grandchildren.

In lieu of flowers, the family requests that memorial contributions in Evelyn's name be made to the Regis Foundation or Sacred Heart Hospital.
