She was predeceased by: her parents, Kenneth Horton and Dorothy Horton (Gallup); her husband Ronald Cook, Sr.; and her siblings, Omer Horton and Patricia.
She is survived by: her children, Allen Quinn (Catherine), Debbie Quinn (David Novitski) and Stephanie Roth (Richard); her grandchildren, Steven Quinn (Elizabeth), David Quinn (Kristin), Jenna Roth and Rebecca Roth; her stepgrandchildren, Brian Novitski, Timothy Novitski, Kevin Novitski and Anastacia Roth; and her great grandchildren, Aubrey of Avril, AJ, Viola, Leon, Kye, Ivy, Charlee and Rucker.
In lieu of flowers, memorials can be made to the family to be given to Hospice Home of Hope in Phyllis' name.
