He was predeceased by: his parents, Glenn Polk and Ruth Polk (Piquette); his daughter Bridget Polk; his sisters, Theresa Streib, Peggy Hill and Anne Andryszcyk; and his brother-in-law Craig Hill.
He is survived by: his children, Molly Polk of Johnson Creek and Regan Polk (Tori) of Bend, Oregon; his grandchildren, Brayden, Christian Polk of Johnson Creek, Eddy and Mays Polk of Bend, Oregon; his siblings, Mary Witte (John), John Polk (Nancy), Judy Weimer (Gary) and Jim Polk (Susie); and his brothers-in-law, Bruce Andryszcyk and Paul Streib. He is also survived by many nieces, nephews, extended family members, and friends.
