He is survived by: his wife Nancy; his children, Todd Cook of Florida, Mike Cook of Florida, Jody Cook of Fox Lake, Nicole Waterworth (Duane Schraufnagel) of Beaver Dam, Amy Cook of Fox Lake, Shane Cook (Erin) of Cambria and Jamie Cook of Fox Lake; his grandchildren, Taylor Giese (Kyle), Mackenzie Raley, Austin Cook, Alyssa Cook, Julia Cook, Kara Cook and Katrina Cook; his great grandchildren, Braxton, Baylee and Brynlee; his siblings, Orville Cook (Ann) and Linda Cook; his brother-in-law Dan Lawton; and his nieces, Peggy Sue Cook, Cindy Sedbrook and Amanda Lawton. He is also survived by many other nieces, nephews, other relatives and friends.
