He was predeceased by: his parents, Carl Polak and Erna Polak (Klein); his wife June Magray (Utech); and his son Chad Polak. He is survived by: his children, Amy Matz of Parker, CO, Dennis Polak (Sheila) of Merrill and Delmer Polak (Tracey) of Merrill; his stepsons, Michael Magray (Kelly) of Merrill and John Magray (Vanessa) of Kewaskum; his grandchildren, Adam, Nolan, Grant, Jack Polak, Drew and Alana Polak; his stepgrandchildren, Alison Pint Stanfield (Josh), Alexis Yost (Alex), Trevor Magray and Aubrey Magray; his stepgreat-grandchildren, Arlo, Lennon, Dottie, Aries and Axel; his brother David Polak (Donna) of Merrill; and his sister-in-law Darlene Rethaber of Merrill.
Visitation was held on Saturday, September 13th 2025 at 9:00 AM at the Luther Memorial Lutheran Church (N5308 Town Hall Rd, Gleason, WI 54435). A funeral service was held on Saturday, September 13th 2025 at 11:00 AM at the same location.
In lieu of flowers, memorials may be made to the Luther Memorial Church Building Fund.
