He was predeceased by: his former wife Maxine Werner (Loritz); his parents, Fred Werner and Lucille Werner; and his siblings, Darrell and Shirley.
He is survived by: his wife Kathy; his children, Stacey Werner (Sheila), Vicky Heimke (Korey) and Jodie Polifka (Roger); his stepdaughters, Pamela Tice (Greg), Jennifer Akre and Kari Noll (Jacob); his grandchildren, Maximas, Philip, Mason, Shawna, Marcel, Joey (Cheyenne), Micah, Bethany, Danielle, Lauren, Caeleb, Ezra, Elias, Henry and Jeremiah; his sisters, Millie, Judy and Carol; Loradon Fischer (Calvin) and Dennis Loritz. He is also survived by many nieces, nephews, other relatives and friends.
In lieu of flowers, the family kindly requests donations be made to a memorial fund established in Dave's name for Grace Christian Academy or Cup O' Joy.
