He was predeceased by: his parents, Michael P. Forslund and Bernice Forslund (Evancheck); and his brother Michael C. Forslund. He is survived by: his wife Cheryl Hebert; his sons, Justin Konyn and Jordan Forslund (Kristi); his granddaughter Michele; and his nephew Michael D. Forslund.
