He was predeceased by: his parents, Frank Falkowski and Jeanette Falkowski; his wife Linda Chase; his siblings, Ron and Sally; and his sister-in-law Sissy.
He is survived by: his son Jay Falkowski (Katherine Bilitzki) of Chicago; his siblings, Frank Falkowski Sr., Robert Falkowski (Martha) and Sandra Blaise (Ed); and his grandsons, Frank and Joseph Falkowski.
Visitation will be held on Saturday, August 9th 2025 from 1:00 PM to 3:00 PM at the Kenosha Funeral Services (8226 Sheridan Rd, Kenosha, WI 53143). A memorial service will be held on Saturday, August 9th 2025 at 3:00 PM at the Kenosha Funeral Services & Crematory (8226 Sheridan Rd, Kenosha, WI 53143).
