He is survived by: his son Jay Falkowski (Katherine Bilitzki) of Chicago; his siblings, Frank Falkowski Sr., Robert Falkowski (Martha) and Sandra Blaise (Ed); and his grandsons, Frank and Joseph Falkowski.
