He is survived by: his wife Sharon Guptail; his sons, Daniel Guptail (Brianna) of Memphis, TN and Matthew Guptail of Stevens Point, WI; his grandson Aiden Guptail of Dubuque, IA; and his siblings, William Guptail (Kathleen) of Huntley, IL and Sandra Sommers of South Elgin, IL. He is also survived by relatives and friends.

In lieu of flowers, memorials to St. Jude Children Research Hospital in Jerry's name would be appreciated.
