She is survived by: her husband Erik Jacobson; her parents, Robert Perkins and Wanda Perkins; her children, Dylan Forrest (Jessie Smith) and Maranda Forrest (Myles Welch); her grandchildren, Remy Jacobson, Dylan Jacobson, Anika Martin and Mylah Welch; her sister Kristina Forrest (Kevin Suckow); her mother-in-law Susan Jacobson; her nephew Robert Holt (Alyssa Feasel); and her niece Shyanne Revels (Dacoda Revels). She is also survived by relatives and friends.
