He is survived by: his significant other Lindsey Lewko; his sons, Thomas and Boston; his mother Patrice Martinez of Mauston, Wisconsin; and his siblings, Deja Lewko of Sun Prairie, Wisconsin, Rusty Lewko of Wisconsin Rapids and Chris Davis of Tomah, Wisconsin. He is also survived by many extended family members and countless friends.
