She is survived by: her husband Bob; her children, Craig Formanek of San Carlos, CA, Danielle Swiggum (Brandon) of Eastman, WI, Krista Clinton (Joel) of Holmen, WI and Ryan Formanek (Lindsey) of Viroqua, WI.; her grandchildren, Gabriela Formanek, Ethan (Elizabeth), Zachary, Sawyer, Cole Swiggum, James, Luke, Gavin Clinton, Ben and Will Formanek; and her siblings, Karen Johnson, Terry Hass (Mary Sue), Diane Cooley, Marilyn Quinn (Doug), Teresa Martin (Ron) and Linda Greene (Terry). She is also survived by many nieces, nephews, and cousins.
In lieu of flowers, the family kindly asks that memorials be made in Patty's honor to Aquinas High School (https://www.aquinascatholicschools.org) or Western Technical College.
