He is survived by: his wife Gloria; his children, Ben Dvorak (Erin) of La Valle, Kelly Rabuck (Randy) of Reedsburg and Brian Dvorak (Jessica) of Hillsboro; his grandchildren, Brandon Tobey (Emily), Daniel Rabuck (Mckenzie), Alma Spry (Hunter), Kenton Dvorak (Kaelyn), Gabby Rabuck, Elly Dvorak, Nick Dvorak, Jonathan Dvorak and Sophie Rabuck; his great grandchildren, Kazleigh Rabuck, Oliver Spry and Calix Rabuck; his sisters, Ellen Bender (Bob) and Helen Anderson (Marvin); and his sister-in-law Barb Dvorak. He is also survived by Aunts, in-laws, nieces, nephews, cousins, and friends.
