He was predeceased by : his father John "Tuck" Forward; and his uncle David "Skip" Forward (Cliff Weber). He is survived by : his mother Beth Forward; his siblings, Jack Forward and Maddie Forward; his aunts, Lynn Rodgers, Laura Davis (Chris), Theresa Rodgers, Jane Rodgers and Julie Bettinger (Mark); his cousins, Sally of Charlie, K. C., Brian, Anastasia, Sophie, Anna and Max; and his grandmother Molly Weber. He is also survived by other relatives and friends.
