He was predeceased by: his parents, Robert Polzin and Barbara Polzin (Harvieux); and his brother Donald Polzin. He is survived by: his daughters, Piper and Payton Polzin; his sisters, Deb Polzin and Denise Hartenstein (Scott); and his siblings' children, Matt (Jamie), Trace' (Brent), Ellen, Laina, Kari (Dave), Alicia (Wayne), Cassie (David), Troy (Sarah), Katie (Brandon) and Kristen.

In lieu of flowers, memorials are preferred.
