She was predeceased by: her close friend Jeff Kuzma; her stepmother Kathleen Phillips; and her stepsister Rae Ellen Packard. She is survived by: her parents, William Phillips of Madison and JoAnn Thrasher of Springfield, Ill.; her siblings, Edward Phillips (Mareea) of Downey, Calif. and Marcy Phillips (Marti Eyman) of Springfield; and her stepsiblings, Bob Rauch (Anita), Randy Rauch, Rick Rauch (Lindsay) and Ruth Ann Weber Gannon (Rob). She is also survived by close friends and family.

Memorial donations may be made in Cindy's name to Our Lady Queen of Peace Parish in Madison.
