Haley Ann Smith Obituary

It is with sadness that the passing of Haley Ann Smith in Madison, Wisconsin is shared. Haley Ann, born on June 27, 1995, passed away on September 14, 2026. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

Haley Ann will be remembered for the time spent with loved ones and the moments that brought joy to those around her. Friends and family will honor Haley Ann by reflecting on shared memories and the positive influence she had in their lives. Though Haley Ann is no longer present, the bonds she created and the experiences shared will endure. Haley Ann’s memory will continue to be a source of comfort for all who knew her.

She was predeceased by: her mother Gail Smith (Berkvam). She is survived by: her father Stephan Smith; her uncles, Stan Smith, Rich Berkvam and Ron Berkvam; and her aunt Debbie Bloomberg (Gordie).

A celebration of life will be held on Thursday, October 1st 2026 from 5:00 PM to 7:00 PM at the Tuschen-Newcomer Funeral Home (302 Columbus St, Sun Prairie, WI 53590).
