He is survived by: his mother Doris Shoulak (Moddie) of Winter Haven, FL; his daughter Brianna Shoulak; the mother of his child Tammy Hollen (Jeff); and his sister Tammie Dose (Paul). He is also survived by aunts, uncles, cousins, other relatives and friends.
