He was predeceased by: his parents, Leonard Kocourek and Evelyn Kocourek (O'Neil); his siblings-in-law, Bill Garceau, George, Corrin Herrmann and Cindy Herrmann; and his nephew Christopher Herrmann.
He is survived by: his wife Mary Jo Kocourek (Herrmann); his children, Dr. Erin Bannink (Dan) and Matthew J. Kocourek (Amber); his sisters, Pat Neuser (Paul) and Nancy Garceau; his brother-in-law Dick Herrmann; his stepchildren, Rob Sieracki (Debra), Jamie Sieracki (Deb), Rick Sieracki and Amy Schmidt (Tom); his stepgrandchildren, Trevor Sieracki, Kinsey Gonzales (Jacob), Shelby Sieracki, Chase Sieracki, Carrie Bratz (Roy), Kyle Sieracki (Eliane), Cody Sieracki (Ersy), Kendra Johnson (Matthew), Sharaya Schmidt and Cheyenne Schmidt; and the mother of his children Joyce Kocourek. He is also survived by eight step great grandchildren, nieces, nephews.
